In order to realise a transformed, sustainable and fully integrated health and social care system, West London CCG is pioneering a whole systems approach to the delivery of care to its older adult population. In practice this means employing both horizontal and vertical integration of care services in health, social care and the voluntary sector. The West London Whole Systems model is designed to integrate care for those over 65, by transforming primary care, creating a community hub and integration across planned, intermediate, urgent and acute care. Attain has been engaged to support the CCG to design and deliver the programme of work and to support the implementation of a new service.

We worked intensively with a CCG in North Yorkshire to create a sustainable system strategy for the local healthcare economy, aiming to achieve integrated, outcomes-based and high quality out-of-hospital care, together with development of both primary and community care.

Integrated out of hospital services – care closer to home

Throughout the past 12 months, Attain has supported the design, development, and implementation of North East Essex CCG’s ambitious ‘Care Closer to Home’ programme, which aims transform the delivery of out of hospital health and social care services across North East Essex for the next 7–10 years.

Care closer to home will lead to more integrated services which provide seamless and patient centred care, based on individual needs and supported by appropriate care planning. Services will re-focus on helping people to stay independent for as long as possible, enabling them to manage long term conditions and supporting them to recover quickly and regain independence following accidents or episodes of ill health.
